What is it?
The Check-Up Assessment (CU) is a self-reported questionnaire that assesses health and social needs. The CU identifies areas of strength, areas to monitor, and areas at risk, and can be used to inform health and wellness goals, identify needs, and communicate concerns with healthcare professionals.
How can it help me?
The CU results can help patients and caregivers by:
- Providing direct access to health information, including areas of strength, areas to monitor, and areas at risk. This information can be used to track and monitor health changes over time.
- Identifying important concerns to discuss with healthcare professionals.
- Accessing recommended YourCare+ resources to support identified care needs.
The CU results can help healthcare providers and caregivers:
- Update care routines and address changes in health.
- Discover unrecognized or unmet needs.
- Prioritize relevant topics of discussion for medical appointments including quality of life, life choices, well-being, and functioning.
Completing the assessment
- It is best to complete the assessment on a desktop or laptop computer instead of a cell phone or tablet.
- It takes approximately 30 minutes to complete.
- The assessment can be completed in multiple sessions. Save progress and take breaks if needed.
- It must be completed within 48 hours of starting to ensure answers are current.
Please consider:
- Completing the assessment may bring up difficult emotions. Protect your emotional well-being by taking breaks or stopping if necessary.
- Results may identify areas where improvements in care are needed. This is not meant to cause guilt or fear. Results are intended to help patients, caregivers, and health professionals identify what supports are needed to improve health.
